Information for support coordinators

Capacity, what we can clinically take, and a clear answer on registration. Written for people who place participants for a living.

You need three things from a provider page: can they take it, can they do it safely, and will they ring you back. Here is all three without the marketing.

Registration status

Vivid Support Network is not yet NDIS registered. The application is in progress. Until the Commission register shows us, we can work with plan-managed and self-managed participants only. If your participant is NDIA-managed, we cannot take the referral until registration is confirmed.

Current capacity

SIL: two rooms in the Glendenning home, available now. The house, in detail, including an honest account of what it cannot accommodate.

In-home supports: open capacity across the Blacktown LGA, typically starting within a week of an agreed schedule.

Nursing and high intensity: open capacity, subject to the clinical assessment below.

What we can clinically take

Clinical governance sits with a Registered Nurse, so complex supports are delivered with clinical oversight in-house rather than subcontracted: complex bowel care, enteral feeding and PEG, diabetes management including insulin, seizure support including prescribed midazolam, urinary catheter management, complex wound management, dysphagia support to a speech pathologist's mealtime plan, and severe psychosocial supports where a behaviour support plan is in place.

We do not take ventilation. Including tracheostomy ventilation.

How we handle a referral

Acknowledgement the same business day. A yes or no on capacity within two business days. Where it is a yes, a meeting with the participant before anything is signed, and a service agreement and quote in writing before supports start.

Where we cannot take a referral, we say why and refer you on.

What we need from you

Plan management type, the funded supports and roughly what is left in them, the supports actually needed, any behaviour support or mealtime management plan, and a realistic start date. If there is a hospital discharge involved, the discharge date and the ward contact.

Are you a registered provider?

Not yet. Registration is in progress. Until it is confirmed we work with plan-managed and self-managed participants, and we will tell you that in the first conversation rather than after a referral.

What is your current capacity?

Two rooms in the Glendenning SIL home, and open capacity for in-home supports across the Blacktown LGA. We confirm capacity the same day.

Which high intensity supports can you take?

Complex bowel care, enteral feeding and PEG, diabetes management including insulin, seizure support, catheter care, complex wound management and dysphagia support to a speech pathologist's plan. Not ventilation.

How fast do you respond to a referral?

Same business day for an acknowledgement, and a yes or no on capacity within two business days.

Do you provide reports for plan reviews?

Yes. Clinical and support documentation for reviews is part of the service, with the clinical sections written by a Registered Nurse.
