Make a referral

For support coordinators, plan managers, hospital teams, families and participants. Acknowledged same business day, decision within two.

Send us what you have. If a detail is missing we will ask rather than hold up the referral.

What we need

Plan management type, the funded support categories and roughly what remains in them, the supports actually needed, and a realistic start date. Where they exist, any behaviour support plan, mealtime management plan or nursing care plan.

For a hospital discharge, the discharge date and a ward contact. For a SIL placement, whether the participant has seen the Glendenning vacancy.

What happens next

Same business day. Acknowledgement, with the name of the person handling it.

Within two business days. A decision on capacity, and where it is a no, a reason and a referral onward.

Within a week. A meeting with the participant, at home or at the house, before any agreement is signed.

Then. A written service agreement and quote, and an agreed start date.

What we can take

Supported independent living, in-home support, personal care, household tasks, community participation, transport, daily living skills, community nursing, and high intensity supports with Registered Nurse oversight.

We do not take ventilation, and we do not provide SDA housing, support coordination, plan management or allied health. Full clinical scope and current capacity.

Questions

Who can make a referral?

Support coordinators, plan managers, local area coordinators, hospital discharge teams, allied health professionals, families and participants themselves. You do not need to be a professional to refer.

What happens after I send one?

You get an acknowledgement the same business day and a decision on capacity within two business days. If we can take it, we arrange a meeting with the participant before anything is signed.

What if you are at capacity?

We say so within two business days and, where we can, refer you to another provider. You will not be left waiting on a maybe.

Can you take NDIA-managed participants?

Not yet. Our registration is in progress, so we currently work with plan-managed and self-managed participants.

Do you take urgent or same-week referrals?

Yes, where we have capacity. Hospital discharges and breakdowns in existing supports are the two we prioritise. Call rather than email if it is urgent.
