Moving house, leaving hospital, leaving school or starting again after something changed. The points where supports most often fall over.
Transitions are where plans fail. Not because the support was wrong, but because nobody was holding the pieces together while everything moved at once.
The NDIS funds this as Assist Life Stage Transition, and it is one of the more " useful and least used line items in a plan.
The transitions we support
Moving house, including into supported independent living. Coming home from hospital, which is the highest-risk transition of the lot. Leaving school and working out what comes next. A change in health or function that means the old arrangement no longer works. A change in who provides support, including changing providers. Bereavement, or a carer no longer being able to continue.
What we actually do
The practical parts. Working out the sequence, making the calls, being at the handover, making sure the new arrangement knows what the old one knew, and being around for the weeks afterwards when the gaps show up.
Hospital discharge is worth calling out. The window between a discharge date being set and someone getting home is short, and it is when supports most often go wrong. If a discharge is coming, tell us before the date rather than after it.
Moving into the Glendenning home
If the transition is a move into supported independent living, this is part of how we do it rather than an add-on: the look through, meeting the household, working out what comes with you, and the first fortnight when everything is unfamiliar. The current vacancy.

What is Assist Life Stage Transition?
A Capacity Building line item that funds support through a change: moving, leaving hospital, leaving school, or a change in circumstances. It covers the coordination and the practical work of getting through it.
Can you help with a hospital discharge?
Yes, and earlier is better. Tell us before the discharge date if you can, because the gap between leaving hospital and supports starting is where things go wrong.
Can you help me change providers?
Yes. Changing providers is a normal thing to do and you do not need a reason. We can talk you through notice periods and timing so there is no gap in support.
What if I am moving into the Glendenning home?
Then the transition support is part of how we do the move rather than a separate service you have to ask for.
Is this the same as support coordination?
No. Support coordination is a distinct funded role, and where you have one we work alongside them rather than duplicating what they do.
